As a federally qualified health center (FQHC), no one will be denied access to services due to inability to pay and there is a discounted/sliding fee schedule available based on family size and income. You can use this tool to estimate what your savings would be on our sliding scale.

1. What is a sliding scale discount?
- The sliding fee scale system was developed to enable all FHC patients to participate in paying for their health care services based on their income. Discounts are offered based on family/household size and annual income.
- Prior to receiving health care services, the patient will be requested to pay a percentage of the total cost of services, according to their financial classification. Patients that are in the 100% sliding category will be asked for the established nominal fee for an office visit.
2. What documentation does the patient need to bring?
- Photo Identification (if available)
- Driver’s License
- Government-Issued ID Card
- Proof of Residence/Address (if available)
- Proof of Income or Financial Support which may include one or more of the following:
- Wages
- If wages are paid weekly, please bring the last 4 current check
- If wages are paid every 2 weeks, please bring the last 2 current check
- If wages are paid monthly, please bring the last current check
- Social Security Disability Income (SSDi)
- Patients who receive government benefits will need the most recent letter received from the Social Security Office or Department of Human Services (DHS) stating their income (benefits received).
- Temporary Assistance for Needy Families (TANF)
- Retirement Benefits
- Unemployment Compensation
- Worker’s Compensation
- Alimony/Child Support (Court-order/DHS)
- VA Benefits
- IRS Form W-2
- IRS Form 1040, 1040A, or 1040EZ

3. Can a patient who declares that he or she has no income qualify for the sliding scale discount program?
- Yes, the patient who declares that he or she has no income can qualify for the sliding scale discount The patient can bring in a signed, notarized document which states and serves as proof of financial support.
4. How many days does the patient have to bring in documentation for a sliding scale application?
- Patients have up to 7 business days to bring in documentation to complete a sliding scale application
